Transfer ownership of Prepaid SIM to my Contract.

Hello All,

I recently bought a MagentaXL mobile plan and now want to transfer a Telekom prepaid SIM that belongs to my friend to my plus SIM plan. I was told I need to fill out a formular for transferring my friend's SIM in my name. I can't find it on the website. Can someone point me to it?
